NMEDA membership is open to dealers, manufacturers and other individuals serving consumers with adaptive mobility needs.

How to Apply for Membership 

1. Dealers: Visit This QAP Dealer Application Link
   Associates / Manufacturers: Download Application Form
   (You will need Adobe Acrobat Reader to view the forms.) 
2. Fax or mail the form (with payment information) to the NMEDA office so that it may be processed.

3. Once your application is approved (usually within 2-3 business days) you will be sent a NMEDA Membership Packet in the mail. Please allow two (2) weeks after submission of your application to receive the packet. You will not be notified by phone or email unless we have questions about your application.
